Developed and led the first structured implementation of the Post-Acute Authorization Module, improving adoption and streamlining workflows. Created a discovery questionnaire, conducted time studies, and designed a scalable implementation framework that is still in use today.
Optimized Home Health Network & Care Coordination
Developed a Narrow Home Health Network to optimize capacity, streamline transitions, and improve patient outcomes. This initiative focused on high-quality providers, efficient discharge tools, and improved acceptance rates, leading to benchmarking, provider forums, and increased CarePort adoption for seamless transitions.
Referral Package Standardization Process
Standardized the referral process within the EMR for Home Health, DME, and Home Infusion services. Developed a structured checklist and training sessions for discharge planners, physicians, and nurses to ensure accurate documentation, streamline transitions, and reduce delays in patient care.
